Steps to follow
1. Complete the screening form for the service you are interested in.
You will be asked to upload pictures of your insurance cards.
IM ketamine treatment is a cash only service.
Spravato can be covered by insurance but requires a prior authorizations. See screening form below that covers most of the insurance requirements/stipulations.
TMS treatment can also be covered by insurance but requires a prior authorizations. See screening form below that covers most of the insurance requirements/stipulations.

2. Complete Intake
Once step 1 is completed, we will verify your insurance and if applicable review records sent and reach out to you. We will send you the rest of the intake paperwork which will include: full medication list current and past, medical history, social history, family medical history and review of systems.
This paperwork is best filled out on a tablet or computer. Or it can be filled out in our office before your appointment.
This step can be completed at the same time as scheduling an intake as long as all consent forms completed, screener accepted, insurance validated and insurance cards uploaded.
3. Schedule an intake appointment
Please arrive 30 minutes early to ensure all medical history and current medications are appropriately recieved, and/or complete in the office.
4. Ongoing Treatment
Depending on service provided we will begin the prior authorization process. for esketamine and TMS it can very as to how long this takes based on insurance. Once insurance approves treatment and in the case of esketamine, medication is delivered. we will schedule your treatment appointments.
For IM Ketamine once treatment is determined to be appropriate, we will begin appointments.
For medication management ongoing appointments will be scheduled based on need, response and treatments selected.
