Become a New Patient

1. Complete the new patient packet to begin treatment.
​

You will be asked to upload pictures of your insurance cards. And in the same form continue to complete full intake packet. 

​​

SPRAVTO currently has an "INTEREST" or wait list. Click below to be added if interested in treatment

​

TMS and SPRAVATO treatment can be covered by insurance but requires a prior authorization.  See screening form below that covers most of the insurance requirements/stipulations.​​​

​

There are some exceptions for insurance coverage, and each service please call for any questions 

​​

We are not credentialed with Kaiser and are not seeing clients with BHSO (Behavioral Health Services Onnly) If you are unsure if you have BHSO (Medicare with Medicaid BHSO) you can contact us and we can look it up. We do accept Medicare and Medicaid otherwise (non-BHSO)

​

We cannot do TMS for Clients with Medicare as Medicare requires this service be done by a Physician and we are a Nurse practitioner ran clinic. Though we can accept Medicare for esketamine and Medication Management.

Due to coverage and network limitations, we are not accepting the following insurances for SPRAVATO: Cigna, Kaiser, Tricare and Medicare (all plans)

2. Complete Intake

In the screening form if you are able to check all of the boxes you can continue to complete the full intake packet.

3. Schedule an intake appointment
​

Once you submit the above form with screening and packet done, we will call to schedule.

​

4. Ongoing Treatment
​

Depending on service provided we will begin the prior authorization process. for Spravato and TMS it can vary as to how long this takes based on insurance.  Once insurance approves treatment and in the case of Spravato medication is delivered. we will schedule your treatment appointments.
